Brief Job Summary Under the direction of the Executive Director, the Community Registered Nurse- Case Manager will work in collaboration with the Agency RN Supervisor, the Residential Support Director and other designated team members to meet the health care needs of the persons served by The Council for Exceptional Children and Adults.
The RN will assess the appropriateness of the consumer considered for potential admission into the residential program, provide 24-hour accountability, and be responsible for the consumer’s nursing care needs.
Additionally, the Community Registered Nurse-Case Manager will be responsible for the clinical oversight of unlicensed personnel and determine the ability to delegate certain functions according to the Maryland Nurse Practice Act, mandatory Developmental Disabilities Administration regulations, and the agency’s standards.
Finally, the Community Registered Nurse-Case Manager will follow quality assurance medical policies and procedures, in conjunction with The Council for Exceptional Children and Adults administrative and supervisory personnel.
Specific Duties and Responsibilities: * Comply with the Maryland State Board of Nursing Practice Act.
* Assess and evaluate the medical appropriateness of potential consumers.
* Share 24-hour on-call responsibility and delegate instructions to unlicensed personnel and Certified Nursing Assistants.
* Complete a nursing assessment and establish goals and objectives * Orientation and on-going staff training programs, as needed.
* Maintain certifications that are necessary for compliance with training requirements.
* Act as a medical liaison, on behalf of the consumer, primary care provider, specialist or other health care providers, parents or families and The Council for Exceptional Children and Adults personnel.
* Perform quality assurance visits * Attend admission and discharge and interdisciplinary team meetings, when indicated.
* Maintain confidentiality (HIPPA) of all residents and promote individual’s rights at all times.
* Follow nursing practice standards/DDA regulation for all nursing interventions and documentation.
Qualifications: This professional must possess a current Registered Nurse license within the State of Maryland or an accepted multi-state license to practice.
This professional must have current CPR training.
Preference will be given to nurses who have at least two years of nursing experience.
More consideration will also be given to an RN who has performed direct work in the field of developmental disabilities.
This professional must have no weightlifting restrictions.
The Community Registered Nurse Case Manager will be expected to travel to the agency’s sponsored residential sites and other outside community settings.
Job Type: Full-time Pay: From $25.
