Develops and maintains a clinical pharmacy service, to include advising physicians on matters pertaining to drug selection and appropriateness.
Works to optimize drug utilization.
Coordinates continuing pharmaceutical education of Pharmacy, Nursing, and medical staff and students.
Serves as a preceptor for pharmacy students and fulfill the requirements of West Virginia University of a Clinical Assistant Professor.
Supervises Pharmacists, Technicians and Students.
MINIMUM QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1.
Bachelor of Science in Pharmacy or Doctorate of Pharmacy degree from an ACPE accredited school of pharmacy.
2.
Active West Virginia Pharmacist License.
PREFERRED QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1.
Board Certification.
EXPERIENCE: 1.
Five (5) years relevant work experience AND Board Certification.
2.
Completion of a PGY1 residency AND Three (3) years of relevant work experience and Board Certification.
3.
Completion of a PGY2 residency AND One (1) year of relevant work experience and Board Certification.
CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position.

1.
Performs all essential duties of a Pharmacy Generalist and a Pharmacy Clinical Specialist competently when needed.
This includes but is not limited to accurate compounding and dispensing activities, monitoring for adverse effects of drug therapy, providing clinical services based on pharmacy driven initiatives, protocols, and policies, participation in medical emergencies, and sustaining the appropriate use of formulary medications.
2.
Participates in multidisciplinary rounds and provides leadership and care for patients in assigned areas of responsibility.
Participates in quality improvement initiatives within the department.
Conducts research and collects data to initiate and support projects and process improvement.
3.
Guides providers and patients on the best practices and established guidelines for care and assures safe and effective use of medications through clinical interventions and active involvement with the medical team.
4.
Participates in the management of acute and chronic disease states, makes appropriate recommendations, and assumes responsibility for the provision of pharmaceutical services in the assigned service area.
5.
Assists nursing and other disciplines in admission and discharge planning needs and patient education related to the appropriate use of medication and/or disease state management.
6.
Provides drug information and educational programs to the medical and nursing staffs, other health care professionals, and pharmacy staff.
7.
Provides pharmacy presence and actively participates in multidisciplinary committees within the hospital.
8.
Monitors prescribing habits and conducts medication use reviews to identify inappropriate use of medications and develops a plan for improvement and multidisciplinary education.
9.
Prepares and presents drug monographs and appropriate use analyses for new formulary additions to the Pharmacy and Therapeutics Committee.
Continually conducts formulary reviews to promote and maintain rational drug therapy and minimize the procurement and use of non-formulary medications.
10.
Participates in multidisciplinary planning, development, and maintenance of hospital policies and clinical pathways.
11.
Assists with the development and implementation of processes to maintain compliance with The Joint Commission (TJC) and other regulatory organizations/standards.
12.
Participates in the development and maintenance of continuing professional development for the clinical staff.
Develops and executes competency standards and learning plans for staff and residents.
13.
Participates in the development and presentation of orientation and training programs for the department.
Assists in the development, presentation, and dissemination of educational material for the hospital.
14.
May maintain requirements for preceptorship of pharmacy students and residents, actively participates in the training and education of APPE students and PGY1 residents as a preceptor and assist with the maintenance of the PGY1 residency program, as applicable per physical work location assignment.
15.
Participates in educational and scholarly activities related to drug therapy to enhance personal knowledge and maintain competency in practice areas.
16.
Assists with the evaluation of pharmacy staff performance of core duties.
Maintains professionalism and remains objective when assessing competency and performance of pharmacy staff.
Assists the pharmacy manager and completes annual performance evaluations of pharmacy staff.
17.
Successfully completes annual clinical competency evaluations and maintains competence required for the current position.
18.
Performs other duties as assigned which support the mission and values of the organization.
Willingly accepts assignments within scope of practice, skill set and level of competence.
Assists in developing and maintaining department work schedule and time off coverage.
19.
Maintains good professional relationships with nursing, medical and other hospital staff in order to facilitate patient care.
20.
Embodies the attributes of a leader within the department and acts accordingly.
Encourages a professional work environment and maintains impartiality and equality within the department.

Ability to bend, stoop, reach and be capable of normal rotation, standing and walking for an 11–12-hour shift.

Must meet Department of Labor’s MEDIUM duty standard (MEDIUM WORK- exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly).

May be required to travel between facilities/buildings.
6.
Ability to lift 40 pounds from floor to waist.
7.
Ability to wear latex or vinyl gloves with frequent changes and hand washing.
8.
Ability to tolerate working with and around noxious odors.
9.
Ability to use computer keyboard, monitor and mouse.
